Course Description:
This training is designed to provide staff persons with the required content and level of writing skill required in order to complete a comprehensive Special Incident Report according to Title 22 Regulations so that they accurately reflect the Incident, the subsequent Intervention(s) and appropriate notification of involved parties.
Course Objectives:
- To learn proper procedures for Special Incident Reporting, including Mandated Reporting requirements for youth, dependent adults and elderly.
- To learn the Nine Components of Special Incident Reports: Who, What, When, Where, Why, How, Injuries, Notification & Follow-up.
- Categories of reportable vs. non-reportable incidents.
- To understand the timelines for reporting Special Incident Reports by telephone and in writing to CCL, DCFS OHCMDM, DCFS CSW, DPO, DMH, PPQA GHM, I-Track web-based system (in L.A. County), Parents, Regional Center, etc.
- To understand the purposes of SIR documentation; provides an accurate description of what happened, communication, a permanent record, protection from misrepresentation of staff performance, informs absent team members, provides information for monitoring and modifying Needs & Service Plans
- Internal agency review and sign off procedures for SIRs.
- Manual Restraints and Special Incident Reporting.
